Initial Evaluation
$175
You will receive a thorough examination at your first visit so we understand what's going on. By determining the root cause, we can help you get rid of whatever it may be for good.
Your initial evaluation will begin with a brief discussion to get to know each other. We will discuss your current status, future goals and deepest “why”, review your past medical history, dive further into your concerns, and perform a full-body assessment and movement analysis. These sessions are in person, 50 minutes and one-on-one with a Doctor of Physical Therapy.
Follow Up Treatment
$100-$175
Your treatment will be individualized to you, therefore treatment will vary between each individual. Follow-up treatment sessions are typically 50 minutes in length. Occasionally, a 30 minute visit is warranted and price is adjusted accordingly.

If your schedule allows, we will meet at our office located inside FORM Fitness for your appointments where we will have access to private space, equipment and Pilates reformers.
Mobile
Busy or want to meet in the comfort of your own space? We are happy to come to your home or office and bring the equipment to you. No need to stress about finding a sitter, taking time away from your clients, or requesting time off work to address your goals.
*Travel fees will apply to any mobile service
Virtual
Depending on your initial evaluation and plan of care, virtual follow-up appointments may be an option for those that don't need hands on treatment.
PAYMENT
Payment is due at time of service.
As a courtesy to others, a 24-hour advance notice is required to change or cancel an appointment without charge. Appointments may be canceled by phone, email, or in person. Patients who provide less than 24 hours notice, or miss their appointment, will be charged for a cancellation fee to the card on file.
The card on file will be charged $50 the first time. The second time and thereafter, you will be charged the full appointment rate.
